Parotid Duct Repair with Suturing and Anastomosis Using Tissue Adhesive

“Objectives:” The most common method of parotid duct anastomosis is suturing. Besides the post-operative complications in suturing method (i.e. anastomosis leakage, fistula formation and infection of maxillofacial spaces), the surgery duration and suturing difficulties are propounded as important problems. We evaluated the efficacy of fibrin glue in parotid duct anastomosis, comparing suturing method in 15 dogs. “Methods:” 15 dogs (with similar race and sex) were studied in this experimental trial. Intravenous Keflin (1 gr) and intramuscular Dexamethasone (8mg) was administered pre-operatively for all cases and no drug was administered postoperatively. Transverse incisions were done in bilateral buccal regions to reach to parotid ducts and resection was performed in similar regions (in right and left sides). Then, the right resected duct was repaired with 7-0 nylon sutures and the left resected duct anastomosis was performed, using fibrin glue. 10 days after operation, clinical evaluations and sialography were used to evaluate the anostomosis repair quality, ductal leakage and fistula.” Results:” Ductal fistula in right side was found in 2 cases (13.33%) and 3 cases had ductal fistula in left parotid duct (20%). Aspiration was positive in right side in 2 cases (13.33%) (vs. 5 positive aspirations detected in left parotid duct (33.33%). Also, right ductal leakage was found in 4 cases, using sialography (26.66%) (vs. 7 left ductal leakage (46.66%)).”Conclusion:” This study suggested that efficacies of fibrin glue and suturing method in parotid duct anastomosis were similar, but using fibrin glue had some advantages such as short duration of surgery and possibility of stent sending out during operation.
