Significance of saliva autoantibodies in diagnosis of patients with pSS

SjÖgren's syndrome(SS) is a systemic autoimmune disease.The detection of autoantibodies in serum is very important when diagnosis is established .The recent study showed that several autoantibodies such as SSA/B，a-fodrin and M3R antibody can also be detected in saliva of patients with SS.But the diagnosis value and its comparing levels of autoantibobies in salivary and serum are still uncertain.Detecting the autoantibodies in saliva of patients with pSS and evaluating its diagnosis value will be helpful in clinic .

Objectives: To investigate the sensitivity and the specificity of antibodies including anti-SSA/B antibodies, a-fodrin antibody and antibody against M3 muscarinic acetylcholine receptors, in saliva of patients with pSS and its correlations with clinical manifestations.

Patients and Methods:

37 patients with pSS were included in this study. They all met the Revised 2002 classification criteria for SS. Control group include 33 healthy controls.the serum, unstimulated mixed saliva and stimulated parotid saliva were collected from two groups . Anti-SSA/B,m3AChRs and -a-fodrin antibodiex in saliva were measured by ELISA assay.

Results:

The sensitivity of anti-SSA and anti-SSB in mixed saliva was 64.86% and 78%, the specificity was 100% and 96.97%respectively, ; while,the sensitivity of anti-SSA and anti-SSB was only 32.43% and 8.11% in parotid saliva respectively, the specificity of anti-SSA and anti-SSB was 96.97% ; The sensitivity of anti-m3AChRs antibody and anti-a-fodrin antibody in mixed saliva was 35.14% and 22% respectively, the specificity of each was 96.97% .

Conclusion:

The sensitivity and the specificity of anti-SSA/B antibody in mixed saliva was very high,and it was of highly application prospect as an diagnosis marker of pSS in future .
