Posterior restorations with a nanohybrid resin composite: 48 months results

Objective: Aim of this study was to evaluate the clinical behavior of the nano hybrid composite Grandio (Voco) compared to the fine hybrid resin composite Tetric Ceram (Ivoclar Vivadent) in class II cavities.

Methods: In a controlled prospective clinical study, 68 cavities (52 mo/od, 16 multi-surface) in 30 patients (mean age 32.9 years) were filled by one dentist in a private practice. In a split-mouth design, 36 teeth were filled with Solobond M+Grandio (etch&rinse), same patients received 32 restorations with Syntac+Tetric Ceram (selective enamel etching). At baseline, after 6, 12, 24, and 48 months the restorations were examined by two investigators according to modified USPHS criteria.

Results: All restorations were available at the five recalls (dropout 0%) and clinically acceptable (failure rate 0%). No significant difference between the materials was evident at all recalls (Mann-Whitney-U test; p>0.05). Between recalls (A baseline, B 6 months, C 12 months, D 24 months, E 48 months), statistically significant differences were detected for the criteria [alpha1/alpha2/bravo in %] marginal integrity A 44/55/1 B 9/87/4 C 9/75/16 D 0/60/40 E 0/35/65, integrity filling A 93/4/3 B 72/15/13 C 53/21/26 D 9/41/50 E 2/24/74, and integrity tooth A 91/9/0 B 94/6/0 C 73/25/2 D 40/47/13 E 27/58/15 (Friedman test; p<0.001). For the criteria surface roughness, color match, and proximal contact the quality of the fillings changed as well (p<0.05). After six and twelve months mainly voids and small chippings were recorded, and after 48 months mainly abrasion and negative step formations for both materials were obvious. No differences were found for the criteria proximal contact and hypersensitivities (p>0.05).

Conclusions: The present data demonstrate the good clinical performance for the nano hybrid composite Grandio and the hybrid composite Tetric Ceram after 48 months. Both materials are recommended for direct class II restorations. Supported by Voco, Germany.
